So #1 UFC heavyweight title contender Alistair Overeem failed a late March surprise drug test administered by the Nevada State Athletic Commission (NSAC) for having a testosterone to epitestosterone ratio level higher than five men of Krypton and is now scheduled to appear before the commission in one week to ask for a license to face champ Junior Dos Santos May 26th. But not to worry, Reem fans: Alistair’s manager assures that his fighter will have a “reasonable explanation” for his unreasonable levels, according to Inside MMA‘s Ron Kruck, via CagedInsider.
“I spoke with Glenn Robinson, manager of Overeem and he says he is very respectful and is hopeful that he will be granted his license and the entire team is very confident that he will be facing Junior dos Santos come May 26th,” Kruck said.
“Now Robinson wanted to make it very clear that despite other reports, Overeem did not test high for testosterone, but rather his testosterone to epitestosterone was off and he has a reasonable explanation to why that is, which he will present to the commission next Tuesday.”
If the UFC hadn’t refused to replace Overeem to this point and rule out most viable possible replacements should he not get licensed — and if the NSAC wasn’t in a tricky position of dealing with their first-ever positive test from an out-of-competition surprise test, coupled with their past openness to allowing therapeutic use exemptions for things like testosterone replacement therapy — we’d say Robinson was nuts for thinking his client would get licensed. But as it stands, who knows?
Overeem can still apply for a TUE, and the commission can grant it. We’ll see what defense he gives and if the commission will buy it, in light of his toying with their emotions regulations before his last fight.

parchy, I don't know. But I'm guessing they are somehow saying that the t to et ratio (which is gotten from a urine test) and the overall t level (which is gotten through a blood test) are different in a substantial way...both are not allowed above certain levels by Nevada though. we shall see

I think the answers provided by Xardas were very good ones. Though an oversimplification, you might consider epitestosterone a by product of endogenus testosterone, which is to say that when testosterone is produced in the leydig cells, a subsequently similar amount of epitestosterone will be present as well. As stated, exogenus tesotosterone will have no effect whatsoever on eiptestosterone levels, thus, injections will result in a higher test/epi ratio. The most likely candidate for why your ratio is 9:1 when your total T is 450, and the others is what 6:1 with a total test of 1220, is that epitest is directly related to how much test you are producing. Having said that, someone on HRT or TRT would most likely produce very little testosterone if any, due to long term exogenus administration, which means that there is also little to no epitestosterone present. In contrast, someone who is running a 12 week cycle of 400-500mg of Test per week, would have much higher epitest levels well into the cycle. Therefore, the ratio would be smaller. For example, If your total test level is 450ng/dl and your T:E ratio is 9:1, then your natural production is around 50ng/dl, with equal epitest levels. So yeah your pretty shutdown. Now, the average total test level for an adult male in their thirties is 400-500. This means that epitestosterone levels would be in the same range. Enter 400-500mg of exogenus test per week and the total test would rise, but the natural production would drop, and with it, the level of epitestosterone. This is not an immediate process however, and depending on how far into the cycle the other user is, it is probable that the ratio would be smaller. In fact, a 6:1 ratio means that natural production levels are roughly cut in half, down to right at 200. Also, 600 is a very high figure for natural production at age 45. Maybe the high end of passing a drug test, but there are most likely very few men in this age bracket with levels so high. In the case that your freind is one of them, it would make it more likely that his results would show a smaller T:E ratio. I think the original question posed here was "Does this make sense?" To sum up, it means that you're shut down pretty hard, but if your taking testosterone injections, you know that. Otherwise, the test result really is of no use, unless your being drug tested.
